I understand that South OC Hybrid Homeschool is not an accredited school. It is an enrichment program with the emphasis being on the core subjects of ELA, Math, Social Studies and Science.

I understand that The homeroom teachers of South OC hybrid Homeschool are credentialed teachers for the state of California but our student teaching aides are not and my child may sometimes be taught by an aide for various small group work.

I understand that I am agreeing to pay a non refundable $175 deposit to hold my child’s place. This fee may be applied to the last month’s tuition of the school year. It is cash only and charter funds can not be used for this deposit. I understand that should I leave before the end of the year I will not receive this deposit back.

I acertain that my child can sit in a classroom in a cooperative manner, transition from subject to subject with ease and is not bothered by classroom collaborations that may be distracting.

I understand that this is an academically focused program, that while not overly rigorous in nature or over taxing in busy work or homework is expected its due diligence its seriousness, completion of assignments and active participation in group projects.

I agree to following curriculum assigned to my student on their off days. And If I do not feel I can complete this due to other obligations I will communicate this thoroughly with their homeroom teacher.

I understand that the philosophy of this hybrid homeschool is Cooperative in nature and that oftentimes classes mix together and that the teachers meet each student where they are at. Which may include moving them up or down in math groups, reading groups and writing groups. But they will always have their homeroom peers for morning meeting, group projects, lunch, recess, PE and elective.

I understand that the teachers in this program will not be working with technology in the classroom although many of the assignments given for at home work are computer based. I agree to spend the time needed with my child sitting side by side with them the first month mastering the technology of the program so they feel comfortable completing this work.

I understand that the entire foundation of this program is small group learning. Our program will remain no more than 18 students in a homeroom to 1 teacher. Our homerooms are combo classes so most of the day students will be doing their core learning with a small group of 6-9 to make sure we can guide and teach students in a manner that results in a full grasp and mastery of concepts taught by years end. It is very important to us that we have aides and parent volunteers to help us with this ratio

I understand that this is a short small group tutoring day that is designed to round out the weekly packet (if any) of assigned work from my childs homeroom class. I understand on this day computers may be used to complete those assignments.

I understand that this day will be used to catch my child up on any work or concepts that they need. Or advance them ahead or expand upon projects that they are participating in.

I understand this is also an elective day and my child will do a portion of small group tutoring and then switch to an elective, have a snack and recess break and repeat that with a different subject and elective.

I would like to volunteer for directing morning drop off. I will arrive at 8 am. Set up the cones and block the south drive. Get out all the playground toys, and the backpack rack. Stand at the north end of the parking lot by the kitchens to help students out of their cars and have their parents sign the drop off/attendance sheet. When students are lined up and ready to head into class I will clean up the morning items and put them away.

I would like to volunteer for morning playground play monitor. I will arrive at 8:30 am. Make sure kiddos hang their bags and lunches on the correct racks and watch them on the playground. I will stay until kiddos are lined up for the pledge and any announcements, and then I will put all yard toys away.

I would like to volunteer to be the am parent aide in the classrooms. Arrival 9 am- 11 am finish (we have 3 slots available for this position)

I would like to volunteer to be the PM parent aide in the classrooms. Arrival 1pm-3pm (includes playground monitoring from 2:30-3pm)

South OC Hybrid Homeschool is voluntary and is a privilege.

I release, absolve and hold harmless South OC hybrid Homeschool , its owner, officers, agents, volunteers or employees from any and all liability for any claim arising out of traveling to/from, participating in, or which is in some manner connected with, the participation of my child(ren) in any program or affiliate.

Students and parents are required to obey all rules and safety requirements, as well as the program standards for respect of persons and property.

I hereby give my approval for the participation of my child(ren) in any and all South OC Hybrid Homeschool programs.

In the event of a medical emergency, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of my child(ren). Permission is also granted to South OC hybrid Homeschool and its affiliates including its owner, teachers, staff and volunteers to provide the needed emergency treatment prior to the child’s admission to the medical facility. I understand that South OC Hybrid Homeschool does not have a nurse on site. In the case any special medical needs, such as regular insulin injections, I may make special arrangements with South OC Hybrid Homeschool staff to administer treatment. In the case of allergies, I must train my child to avoid sharing food (South OC Hybrid Homeschool discourages this practice, but we cannot be responsible if a child accepts a food item to which he/she is allergic.) This release is authorized with the purpose of authorizing medical treatment for the protection of my child(ren) in my absence.

South OC Hybrid Homeschool often takes photographs or videos of child(ren) during its classes, activities and events. I grant permission without compensation that these photographs or videos may be used in to promote south OC Hybrid Homescool Academy. south OC Hybrid Homescool agrees to not identify me or my child(ren) by name, or release any other personal information without additional written permission from me.

I realize that I always have the right to drive my own children to/from field trips and/or after-school programs and camps. In the event that I do not pick my child up on time or do not provide my own transportation to an event, I hereby grant permission for South OC Hybrid Homeschool staff, owner, employees and volunteers to transport my child; there will be no charge for this transportation.

I hereby enroll my child(ren) in South Orange County Hybrid Homeschool for the term Aug . 20, 2019 until Dec 18, 2019. I may pay the balance in full or else pay in installments, due on the 15th of each month. We accept cash, credit cards, check, PayPal, and charter funds. Should there be a delay in processing charter funds, I understand that I am responsible for making payments until such charter funds arrive, at which time I am entitled to a refund credit for any overpayment. I agree to pay the tuition and fees per school year as indicated herein. I understand and acknowledge that South OC Hybrid Homeschool is a learning center of limited enrollment, and that in accepting this enrollment and tuition agreement form together with the non-refundable registration fee and tuition deposit, South OC Hybrid Homeschool. agrees to provide a place for my child(ren) for the aforementioned term. I understand that a late fee of $100 on any past due balance shall be charged for any payments not made by the 25th of the month due per the designated installment plan. I also understand that the returned check fee is $25 per returned check or failed transaction. I further understand and agree that the registration fee, books & supplies fees and the initial tuition deposit are non-refundable. I understand that South OC Hybrid Homeschool may offer to accept applicable instructional funds on behalf of charter schools for my child(ren), but that I remain financially responsible for all fees if/when charter funds are not successfully obtained for any reason. Once charter funds are deposited by South Oc Hybrid Homeschool and there is overpayment on my account, Soth OC hybrid Homeschool will issue me refund. The undersigned parents or guardians hereby agree that South OC hybrid Homeschool shall at all times have the right with or without cause in its sole discretion to dismiss or expel the above-named student(s) or refuse to enroll such student(s) for any succeeding school year. No reduction or reimbursement is made for the withdrawal, dismissal, or absence of a student during the term for whatever reason, excepting the death or incapacity of my child(ren). I further specifically agree to waive any claim against South OC Hybrid Homeschool for any losses or damages in any way associated with South OC hybrid Homeschool’s refusal to enroll the above named student(s) for any succeeding school year. In the event it becomes necessary to institute legal action to reinforce the terms of this agreement, I agree that the matter shall be determined only by binding arbitration, according to the rules of the American Arbitration Association and, in the event that the academic program prevails in such action, I will pay the reasonable costs of that action, including reasonable attorneys fees incurred. By my signature(s) below and on the attached contract, I certify that I have read and understand the terms stated in this agreement and its addendum and agree thereto.
